Clear Aftercare
THE SHORT VERSION
A baby tooth may need to be removed when it’s too decayed or infected to save, has been injured, or is stuck and blocking the adult tooth behind it. We numb the area thoroughly, offer nitrous oxide to keep anxious kids relaxed, and walk your child through it calmly. If a back tooth comes out early, we may recommend a space maintainer so the adult tooth has room to come in straight.
When Does a Baby Tooth Need to Come Out?
We always try to save a tooth first, but a few situations call for an extraction. The most common is decay or infection that’s gone too far to repair with a filling or crown. Others include a tooth badly broken in a fall, an abscess that won’t clear, a stubborn baby tooth that won’t loosen on its own and is steering the adult tooth off course, or a tooth that needs to be removed to make room as part of orthodontic treatment.
Whatever the reason, we’ll explain why removal is the better path and what comes next, so the decision is clear to you.
What The Visit Is Like
We start by getting the area completely numb, using a numbing gel before the anesthetic so your child barely feels it. For kids who are nervous, nitrous oxide takes the edge off and wears off quickly afterward. The tooth itself usually comes out faster than children expect — there’s pressure but no sharp pain — and we narrate it in friendly terms so nothing feels scary.
Aftercare At Home
Your child will bite on gauze for a little while to let a clot form, and we’ll send you home with simple instructions: soft foods for the rest of the day, no straws or vigorous rinsing for the first day, and children’s pain reliever if needed. A little oozing is normal at first. Call us if there’s heavy bleeding, growing swelling, or pain that worsens after the second day.
Protecting The Space
Baby teeth hold the place for the adult teeth coming in behind them. When a back baby tooth is lost well before its time, the neighboring teeth can drift into the gap and leave the adult tooth no room. In those cases we’ll often recommend a space maintainer, a small appliance that keeps the spot open until the permanent tooth arrives.
We accept Medicaid and most plans, with CareCredit and Sunbit available — see the Raleigh insurance page.

Frequently Asked Questions
When does a baby tooth need to be extracted?
When it's too decayed or infected to save, has been badly injured, has an abscess, or is stuck and blocking the adult tooth. We always try to save a tooth first and only remove one when it's the better option.
Does pulling a baby tooth hurt?
We numb the area thoroughly with a numbing gel and local anesthetic, so your child feels pressure rather than pain. Nitrous oxide is available to help nervous kids relax during the visit.
What's the aftercare?
Your child bites on gauze to form a clot, sticks to soft foods that day, and avoids straws and hard rinsing for the first 24 hours. Children's pain reliever helps if needed. Call us if bleeding is heavy or swelling grows.
Will my child need a space maintainer afterward?
Sometimes. If a back baby tooth comes out well before the adult tooth is ready, we may recommend a space maintainer to keep the gap open so the permanent tooth has room to come in properly.
Can you keep my anxious child calm for an extraction?
Yes. We use a gentle, explain-as-we-go approach and offer nitrous oxide, and we're experienced with children who have dental anxiety or special healthcare needs.
Does insurance cover extractions?
Extractions are commonly covered by Medicaid and most dental plans when they're necessary. We confirm your coverage beforehand and offer CareCredit and Sunbit for any remaining balance.
